Job Description

Receives pending referrals and palliative consults. Screens for appropriateness. Collaborates with Palliative Care Team and triages resources accordingly. Maintains routine communication with ICU teams. Educates staff, physicians, patients and families. Facilitates transitions to next level of care. Collects, reports and monitors key performance indicators of Palliative Care services.

Job Requirements

Applicable Experience:
3-5 years

Registered Nurse (KS) - National Council of State Boards of Nursing (NURSYS), Registered Nurse (MO) - National Council of State Boards of Nursing (NURSYS)

Bachelor's Degree
