Work Environment
This position is entirely remote, allowing you the flexibility to balance your professional and personal life effectively. Key Responsibilities
- Conduct thorough clinical assessments to identify patient and family needs, establishing personalized treatment plans based on evidence-based practices.
- Implement solution-focused, therapeutic interventions that address the emotional, psychological, and social needs of patients and their families, utilizing innovative techniques and approaches.
- Maintain comprehensive, accurate, and timely clinical records that comply with industry regulations and organizational standards, ensuring the highest level of quality and confidentiality.
- Engage in regular case consultations and multidisciplinary team meetings to discuss patient progress, share insights, and enhance care strategies.
- Provide education to patients and family members on coping mechanisms, resources, and mental health literacy, empowering them through knowledge and support.
- Stay abreast of advancements in social work practices and hospice care to continually improve service delivery and patient care outcomes.
Patient Population / Client Focus
The LCSW will primarily work with terminally ill patients and their families, addressing their complex emotional and psychological needs during a critical period of life. Our patient population is diverse, requiring sensitivity to cultural differences and individual circumstances. The role involves supporting families in understanding the dying process, facilitating difficult conversations, and helping them navigate grief and loss.
Qualifications
- A Master's degree in Social Work from an accredited school.
- A valid LCSW license in the state of New York.
- A minimum of three years of clinical experience, particularly in hospice, palliative care, or related mental health fields is essential.
